Southwest Bridge Health delivers free, primary healthcare to uninsured individuals in high need communities within Southwest Michigan. Through strategic partnerships and community collaboration, we make health access simple, equitable, and local.

Why We Exist

Many Southwest Michigan residents fall into the coverage gap—earning too much to qualify for assistance but not enough to afford insurance or out of pocket care. These barriers lead to untreated chronic conditions, preventable hospitalizations, and avoidable suffering.

Southwest Bridge Health exists to change that by offering trusted, community embedded care that meets people where they are.
